- Who is the holder of the insurance? If it is somebody other than the insured (the person calling for the appointment) then you will need the following: Insured’s name, DOB, SSN#, address, employer & insurance phone#

CHILD AND ADOLESCENT PATIENTS
- Is the patient a State Ward? If so, the caseworker must call in to schedule the appointment and give verbal consent OR we can fax documents needing to be signed by the caseworker.
- Step parents can not set up the appointment nor sign any documents at the appt.
- Adoption – we need a copy of the paperwork that shows a patient has been adopted and “who” the legal guardian is.
FOR ADULTS who have a GUARDIAN or POWER of ATTORNEY--
- Caregivers, who are Guardians or Power of Attorneys for Adult patients, must provide us the legal documents at the 1st appointment – we keep a copy in the chart.
